User Setup Forms

Information Services offer several types of user account setups. The user setup forms below should be completed by, or on behalf of, a Line Manager, or appropriate departmental authority.

Use this form if you have a new HR Permanent/Fixed Term contracted member of staff joining the University;

Use this form if you have an existing GCU member of staff joining you from a different department within the University;

Use this form if you have a new guest user who is contracted via a local departmental budget/contract;

Use this form if you have a PhD student who needs setup with staff associations;
